To participate in any sport at St. Catherine of Siena Catholic School, the following forms must be completed and turned into the Athletic Director before participating in practice or games:
  1. Sports Waiver
  2. Concussion Form
  3. NC Sports Participation Form
  4. Signature page of the SCS Athletic Handbook - page 19 is the Athletic Handbook Parent Signature Page
