Welcome to the 2021-2022 Admission Process!

Thank you for applying to St. Catherine of Siena Catholic School for the 2021-2022 school year.

Only one parent from each family needs to complete these tasks.

K - 8 NEW FAMILY (PREK Scroll to the bottom):

STEP 1: Please complete the Online Application

STEP 2: Please complete the Tuition Payment Preference Form

STEP 3: New Families: Set up a FACTS Account for scheduling tuition payments unless you are paying in full

STEP 4: Please complete the Catholic Parish Affiliation Tuition Rate Form (if applicable)

STEP 5: Please gather/complete and submit the following to the school office:

Required documents:

Letter of Release for Student Records (transfer students) and return to SCS. Print out, complete form, scan or take a picture and return to ipesa@scswf.org.
Confidential Teacher Recommendation Form (Grades 1 - 8) to your child's past/current teacher (transfer students). Print or save electronically and give/send to your child's current teacher.
Send Confidential Teacher Recommendation Form for Rising Kindergarteners (K if previously in PreK). Print or save electronically and give/send to your child's current teacher.
Submit a copy of Birth Certificate and Immunization Records (scan or take a picture - can be submitted in person or by email to ipesa@scswf.org)
Submit a copy of Baptismal Record and First
Communion (if applicable, scan or take a picture - can be submitted in person or by email to ipesa@scswf.org)

All forms must be completed, submitted and the application fee paid to move to the next step which is an assessment scheduled.
