Kindergarten | Health Assessment Form & Vaccine Requirements

Dear Parents,

Welcome to Kindergarten! Attached is the Kindergarten Health Assessment Form , which should be completed by your child's physician and returned to the front office along with an updated record of immunization by the first day of school.

As per the State of NC, rising Kindergarten students are required to have the immunizations listed below before school entry.

Please contact your healthcare provider if your child needs these vaccines. For more information: https://immunization.dph.ncdhhs.gov/schools/.

Parents, please provide an up to date vaccine record for your rising kindergarten student by the first day of school. If you have already provided a copy to the front office, thank you!

Vaccination requirements for kindergarten-age children can be found below:

Diphtheria, tetanus and pertussis	5 doses
Polio	4 doses
Measles	2 doses
Mumps	2 doses
Rubella	1 dose
Haemophilus Influenzae type B (Hib)	4 doses
Hepatitis B (Hep B)	3 doses
Varicella (chickenpox)	2 doses
Pneumococcal conjugate	4 doses
